Introduction: Few studies have reported the amount of physical activity (PA) and its associations to physical performance of warfighters during military field training (MFT). The purpose of this study was to investigate changes in neuromuscular performance and PA among male Finnish Army conscripts during a 21-d MFT and to evaluate their recovery during 4 d after MFT. Methods: Body composition and physical performance were measured four times during the study (before MFT (PRE), after 12 d (MID), post training (POST) and after 4 d of recovery (RECO)). PA was measured throughout MFT in a group of healthy young male conscripts (n=49) by using a tri-axial accelerometer. The study was approved by the Finnish Defence Forces and was granted ethics approval by the Ethics Committee of the University of Jyväskylä. Results: Body mass declined significantly from 73.5 ± 8.7 to 71.6 ± 8.2 kg, but it recovered close to the PRE values (73.0 ± 8.3 kg). The same trend was also found in skeletal muscle mass and fat mass. The change in body mass and in skeletal muscle mass correlated negatively with the change in vigorous physical activity (r = −0.374, p = 0.016, and r = −0.337, p = 0.031, respectively). Muscular endurance decreased significantly (p < 0.001) in sit-ups from the PRE (46 ± 9 reps/min) values compared with MID (40 ± 8 reps/min), POST (42 ± 8 reps/min), and RECO (34 ± 11 reps/min) values. Also in push-ups, the declines in the POST (34 ± 10 reps/min) and RECO (34 ± 13 reps/min) values (p < 0.001) from the PRE (40 ± 13 reps/min) and MID (39 ± 12 reps/min) values were observed. There was a significant decrease in a standing long jump in all measurement points MID (220 ± 20 cm), POST (216 ± 20 cm), and RECO (213 ± 20 cm) as compared with the PRE values (229 ± 23 cm, p < 0.001). There was no change in 3.2 km loaded march time between the PRE (23:57 ± 4:12 min:s) and POST (23:44 ± 5:02 min:s) measurement time points. In PA, the total number of steps per day was significantly (p < 0.001) greater during ST (13,722 ± 2,379 steps) and MFT (13,937 ± 2,276 steps) than during garrison days (9,550 ± 2,569 steps). In POST, there was significantly (p < 0.001) more light (1.5–3.0 metabolic equivalent) (2:34:38 ± 0:22:53 h:min:s in ST and 3:03:27 ± 0:23:24 h:min:s in MFT) and moderate (3.0–6.0 metabolic equivalent) (2:12:15 ± 0:23:14 h:min:s in ST and 2:47:59 ± 0:27:23 h:min:s in MFT) PA than in the PRE measurements. Conclusion: This study demonstrated slight decrements in warfighter physical performance during the 21-d MFT. The conscripts were overloaded during MFT, but 4 d of recovery seemed not to be enough to obtain the PRE measurement values in physical performance. This study also showed changes in the muscular endurance levels and PA during the 21-d MFT. It is important for warfighters to have a good physical fitness level PRE training or combat. As the prolonged MFT may have adverse effects in warfighters muscular endurance and PA levels, it is important to have sufficient recovery time after long MFT to regain combat readiness. ; peerReviewed

Objectives: Generally, operational military duties are associated with a variety of stressors, such as prolonged physical activity (PA). However, limited information is available on the occupational workload or changes in PA during international military operations. Thus, the aim of the study was to investigate the changes in body composition, stress biomarkers, PA, and heart rate (HR) responses of 79 male soldiers during a 6-month international crisis management operation. Material and Methods: Measurements were conducted 3 times in South-Lebanon during the operation. Body composition was assessed by the bioelectrical impedance method. Blood samples were analyzed for serum testosterone, sex-hormone binding globulin (SHBG), cortisol and insulin-like growth factor. Saliva sampling was used for analyzing stress biomarkers, cortisol and α-amylase. Heart rate and physical activity were monitored by a recordable belt and tri-axial accelerometer, respectively. Results: Increases in muscle mass (39.2±4.1 vs. 39.5±4.2 kg, p < 0.05) and testosterone (15.9±4.6 vs. 17.2±4 nmol/l, p < 0.01), and reductions in PA variables (e.g., daily step count 9472±2547 vs. 8321±2720, p < 0.05) were observed during the first half (i.e., PRE-MID) of the study. The increase in muscle mass remained significant during the latter half (PRE-POST, 39.2±4.1 vs. 39.6±4.4 kg, p < 0.05), but also fat mass increased (MID-POST, 10.6±4.6 vs. 11.0±4.7 kg, p < 0.05) while SHBG (MID-POST, 31.8±12.1 vs. 26.6±13.2 nmol/l, p < 0.01) and cortisol (MID-POST, 445±116 vs. 400±123 nmol/l, p < 0.05) decreased. With the exception of increased concentration of salivary α-amylase (PRE-POST, 36.5±33.7 vs. 55.1±39.7 U/ml), the acute stress biomarkers and HR responses remained unchanged. Furthermore, the low quantity of PA, low HR values and subjective ratings of exertion refer to rather light physical workload. Conclusions: Due to the operatively calm nature of the working environment, the present soldiers did not express any significant signs of physical overload during the study period. ; peerReviewed

Purpose: The purpose of the present study was to investigate changes in physical fitness and anthropometry of young men entering the military service in Finland during the years 1975–2015. Methods: The study included the fitness test results of 627,142 healthy young male conscripts (age 19.1 T 0.4 yr). Data included results of aerobic capacity, muscle fitness tests, and anthropometric characteristics. Results: The results show that the increase in mean body mass of young men has slowed down during the last 10 yr. However, the total increase in body mass was 6.8 kg (8.8%, P e 0.001) between 1993 and 2015. The mean distance achieved in the 12-min running test decreased by 337 m (12.2%, P e 0.001) between the peak in 1980 and 2015. The relative number of conscripts who ran less than 2200 m increased from 3.6% to 25.9% (P e 0.001) between 1980 and 2015, and the proportion who ran more than 3000 m decreased from 25.1% to 6.5% (P e 0.001). The relative number of conscripts who achieved an excellent or good muscle fitness index decreased from 66.8% to 40.1% (P e 0.001) between 1992 and 2000, and remained unchanged between 2000 and 2010. However, the proportion who achieved a poor muscle fitness index increased from 8.1% to 31.4% (P e 0.001) between 1992 and 2010. Discussion: The present study shows that the increase in mean body mass of young male conscripts has slowed down during the last 10 yr. However, their aerobic capacity has still decreased during recent decades. In addition, the proportion of conscripts with poor muscle fitness has increased. From the national defense and health perspective, more initiatives are needed to encourage young men to increase their level of daily physical activity to be fit and ready for operations. ; peerReviewed

This article describes a protocol for an intervention using an interactive accelerometer smartphone application, telephone counselling, and physical activity recordings to increase the physical activity of workers in the military and improve their health. Under the protocol, employees from six military brigades in Finland will be randomly assigned to intervention and control groups. The intervention group's participants will use accelerometers to measure their daily physical activities and their quality of sleep for six months. They will receive feedback based on these measurements via a smartphone application. The intervention group's participants will be encouraged to exercise for two hours per week during working hours, and to participate in telephone counselling. The control group's participants will continue with their normal exercise routines, without the accelerometer or feedback. The participants of both groups will be measured at the baseline, after the intervention period, and six months after the end of the intervention. The measurements will include accelerometer recordings, biochemical laboratory tests, body composition measurements, physical fitness tests, and questionnaires on sociodemographic factors, physical activities, and health. The primary outcomes will indicate changes in physical activity, physical fitness, and sick leave absences. The findings will help to develop a straightforward and cost-effective model for supporting the health and working capabilities of employees in the military and other workplaces. ; peerReviewed

Introduction: Optimal diet together with good physical fitness maintains readiness and military performance during longer deployments. The purpose of this study was to describe changes in dietary macronutrient and energy intake, total physical activity and body composition during a 6-month deployment in South Lebanon. Furthermore, associations of diet macronutrient intake and physical activity on body composition were also studied. - Materials and Methods: Forty male soldiers kept a 3-day food diary and their body composition was measured via bioimpedance and ultrasonography. Total physical activity was evaluated by accelerometers in a subgroup of participants. Measurements were conducted in the PRE-, MID-, and POST-deployment. - Results: Mean carbohydrate intakes were 39.5–42.6 E%, protein intakes 18.7–22.3 E%, and fat intakes 34.9–35.7 E%. Daily energy intake remained stable (10.1–10.3 MJ/D). Total physical activity was decreased during deployment (e.g., step count from 9,835 ± 2,743 to 8,388 ± 2,875 steps/day, p = 0.007). Skeletal muscle mass and subcutaneous fat increased by 1.3% (p = 0.019) and 1.9% (p = 0.006), respectively. Energy and fat intake associated positively with body mass and skeletal muscle mass (r = 0.31–0.48, p < 0.05–0.001). - Conclusions: Carbohydrate intakes and physical activity were low, compared with the general recommendations. Protein intakes were relatively high. Skeletal muscle mass and subcutaneous fat increased. Suboptimal diet together with low level of physical activity may have a negative impact on body composition, physical performance, and cardiometabolic health. Consequently, soldiers should be encouraged to consume more fiber-rich carbohydrates and less saturated fatty acids as well as maintain a high level of physical fitness to sustain military readiness during long-term deployments. ; peerReviewed

Objectives The purpose of the present study was to investigate how aerobic fitness, muscle fitness and body mass index (BMI) change in relation to their baseline levels during 6-12 months of military service. Design Retrospective longitudinal follow-up study. Methods The study group consisted of 249 279 healthy young male conscripts (age 19.1 ± 0.4 yrs.) who completed their military service between the years 2005-2015. Anthropometrics (body mass, height, BMI), aerobic fitness (12-minute running test) and muscle fitness (sit-ups, push-ups, standing long jump) were measured. Results A 12-minute running test improved by 5% (107 ± 292 m), standing long jump 1% (2.1 ± 16.2 cm), 1-min sit-ups 19% (4 ± 8 repetitions/min) and 1-min push-ups 33% (5 ± 10 repetitions/min) (p < 0.001 for all). Baseline fitness and baseline BMI levels were inversely associated with their changes (r = −0.37 - −0.47, p < 0.001). Performance improved in conscripts in the lowest two baseline fitness quartiles in all tests, while it decreased in conscripts in the highest fitness quartiles. In addition, in conscripts who were obese at baseline, body mass decreased on average by 4.9 ± 7.0 kg (p < 0.001). Conclusions On average, the physical fitness of conscripts improved during their compulsory military service. In particular, conscripts with a lower baseline fitness level or higher BMI showed the largest improvements, which may be significant findings from both a military readiness and national health perspective. However, the decline in physical performance of high-fit conscripts highlights the importance of individualization of physical training and military training load during military service. ; peerReviewed

BACKGROUND: Despite numerous studies providing evidence for positive effects of physical activity and physical fitness, evidence for association between physical fitness and health-related quality of life (HRQoL) in young adults is limited. The aim of the present cross-sectional study was to investigate the association of cardiorespiratory and muscular fitness with HRQoL from the perspective of its physical and mental components among young adult Finnish males. METHODS: The sample consisted of 754 men, with the mean age of 26 years (SD 6.7 years), who participated in the military refresher training. HRQoL was measured using the Finnish RAND 36-item health survey. Cardiorespiratory fitness was determined by a bicycle ergometer test, and muscular fitness by various tests measuring maximal strength and muscular endurance. Logistic regression modelling was used to compare low, moderate and high physical and mental component of HRQoL scores to the respective levels of muscular and cardiorespiratory fitness. RESULTS: The findings of the adjusted (age, educational level, marital status, employment status, smoking, use of alcohol and BMI) analysis showed that cardiorespiratory and muscular fitness are positively associated with both physical and mental components of HRQoL. In terms of the physical component of HRQoL, even a moderate fitness level was positively associated with better HRQoL. In terms of the mental component of HRQoL, the impact was seen only in the group with the highest fitness level. CONCLUSIONS: The findings suggest a positive contribution of physical fitness to mental health and highlight the importance of both muscular and cardiorespiratory fitness in the promotion of HRQoL. Even lighter forms of physical activity that result in moderate physical fitness could contribute to the physical component of HRQoL. In terms of the mental component of HRQoL, higher levels of physical fitness may be needed to gain higher levels of HRQoL among young males.

Importance. High physical fitness is associated with a reduction in risk of cardiovascular diseases and death, but the underlying mechanisms are insufficiently understood. Objective. To determine how aerobic fitness and muscular strength are associated with serum metabolome measures. Design, Setting, and Participants. This cross-sectional study included Finnish men receiving military refresher training from May 5, 2015, to November 28, 2015, representing partly overlapping groups of individuals with the lowest vs highest aerobic fitness and the lowest vs highest muscular strength. Data analyses were conducted from January 1, 2018, to May 31, 2019. Main Outcomes and Measures. The associations of aerobic fitness (determined with maximum oxygen consumption in milliliters per minute per kilogram, measured with maximal cycle ergometer test) and muscular strength (determined with a maximal strength test for lower extremities in kilograms) with 66 metabolome measures from fasting serum samples (nuclear magnetic resonance–based metabolomics) were analyzed. Results. Participants included 580 Finnish men (mean [SD] age, 26.1 [6.5] years). Including overlap between groups, there were 196 men in the lowest aerobic fitness group and 197 men in the highest aerobic fitness group as well as 196 men in the lowest muscular strength group and 197 men in the highest muscular strength group. Of 66 studied metabolome measures, 48 differed between high vs low aerobic fitness groups, including small very low-density lipoprotein (standardized median difference, −0.67; 95% CI, −0.83 to −0.49), large high-density lipoprotein (standardized median difference, 0.89; 95% CI, 0.69-1.15), total triglyceride levels (standardized median difference, −0.52; 95% CI, −0.65 to −0.34), isoleucine (standardized median difference, −0.37; 95% CI, −0.55 to −0.16), leucine (standardized median difference, −0.55; 95% CI, −0.72 to −0.34), phenylalanine (standardized median difference, −0.54; 95% CI, −0.71 to −0.32), glycerol (standardized median difference, −0.64; 95% CI, −0.81 to −0.48), and glycoprotein (standardized median difference, −0.78; 95% CI, −0.95 to −0.62) concentration, a high unsaturation degree of fatty acids (standardized median difference, 0.59; 95% CI, 0.42-0.81), and apolipoprotein B to apolipoprotein A1 ratio (standardized median difference, −0.88; 95% CI, −1.08 to −0.67). Adding aerobic fitness into the regression model after age, education, smoking, use of alcohol, and dietary factors accounted for more than an additional 5% of variation for 25 metabolome measures (R2 range, 5.01%-15.90% by measure). With these 2 criteria, maximal muscular strength was not associated with any metabolome measures. Aerobic fitness was associated with high large high-density lipoprotein particle concentration (R2, 14.97%; 95% CI, 10.65%-20.85%), low apolipoprotein B to apolipoprotein A1 ratio (R2, 14.49%; 95% CI, 10.58%-19.51%), and low glycoprotein concentration (R2, 15.90%; 95% CI, 11.22%-21.51%). Aerobic fitness was also associated with low very low-density lipoprotein, triglyceride, isoleucine, leucine, phenylalanine, glycerol, and glycoprotein concentrations and with a high unsaturation degree of fatty acids. Adjusting for recent physical activity influenced the results minimally. Adjusting for body fat percentage showed that some of the associations were mechanistically associated with body fat percentage. Conclusions and Relevance. This study provides data on the association of high aerobic fitness with underlying oxidative lipid metabolism associated with a reduction in cardiometabolic risk. High maximal muscular strength is not similarly associated with these benefits. ; peerReviewed

Purpose: To examine whether retrospectively assessed sports participation (SP) and competitive sports (CS) at the age of 12 years is associated with mental health and health behavior in young adulthood among males. Design: A cross-sectional study using self-administered questionnaires. Setting: Conducted prior to compulsory military refresher training course in Finland allowing geographically representative sample of Finnish young men. Participants: Six hundred eighty males aged between 20 and 35 years. Measures: Mental well-being was measured with the short version of Warwick-Edinburgh Mental Well-Being Scale and mental distress with 5 items of The Short Form Helalth Survey (SF-36) scale. Analysis: Binary logistic regression models. Results: Sports participation at the age of 12 was associated with better mental health in young adulthood, with both mental well-being (odds ratio [OR] = 1.86, 95% confidence interval 1.11-3.11) and mental distress (OR = 0.61, 0.41-0.90). Age, years of education, and current physical activity were controlled. Higher level of intensity of SP and the level of CS in childhood were associated with lower level of mental distress in adulthood. No association was found between the level of CS in childhood and mental well-being in adulthood. Further, youth SP seemed to be a risk factor for increased alcohol consumption and use of snuff in adulthood. Conclusions: Despite negative outcomes related to health behavior, the findings provide support for the association between youth SP and positive mental health outcomes in adulthood among males.

Healthy lifestyle behaviors including physical activity (PA) have been recognized to contribute positively to mental health. Most of the evidence on relationship between PA and mental health relies on self-reported PA results. Device-based measures on PA or sedentary behavior (SB) are less frequently used in mental health research. The present study aimed at examining the relationship between mental health and PA/SB measured by accelerometers in young Finnish men. The sample consisted of 409 men (mean age 28 ± 7 years), who participated in the military refresher training in Finland. Self-rated mental health was measured with Mental Health Inventory (MHI-5) and short Warwick-Edinburgh Mental Well-being Scale (SWEMWBS) measuring mental health both from the perspective of mental health problems and mental well-being. PA was measured with accelerometer from the perspective of light, moderate, vigorous, and total activity, as well as SB. Linear regression models and compositional analysis were applied. Age, education, marital status, employment status, BMI, alcohol use and smoking were used as covariates. Evidence on relationship between total PA (standardized regression coefficient 0.340; 95% CI 0.022–0.657, p = 0.036) and SB (standardized regression coefficient −0.340; 95% CI −0.657 to −0.022, p = 0.036) with symptoms of mental health problems was found after adjusting for age, education, marital and employment status. The relationship was marginally significant (p = 0.056) after adjusting also for BMI, alcohol use and smoking. No evidence on relationship between PA or SB and mental well-being was found, neither in standard linear regression analysis nor in compositional approach. In our sample of young adult men, PA seemed to have a stronger relationship with symptoms of mental health problems rather than with mental well-being. The findings lead to a conclusion that all PA per se may not be independently associated with mental well-being in young adult males and raise the question whether the domain of PA and its context play a critical role in these relationships. ; publishedVersion ; Peer reviewed
